    Introduction

    Health care systems in India started with the British rule. In the beginning, the purpose of health care was to serve the soldiers and European civil servants and the control of epidemics like plague, cholera, rabies was started with the help of local self-government bodies preferably in the cantonment areas. The existing traditional medicine and Ayurvedic treatment were neglected due to the introduction of Western medicine under the British rule. Initially, treatment services were provided through hospitals and clinics in big cities. In 1940, the Planning Committee suggested providing health services in rural areas and organized training of health workers to provide health services at the rate of 1 health worker per 1000 population. The first Primary Health Center was started in 1942 at Shingur near Calcutta (West Bengal). It was called Rural Health Team. Rural Health Service Dr. James Grad, Director, All India Institute of Sanitation and Public Health has implemented this program.

    Around that time, the health care system was started in the state of Maharashtra with the financial support of the Bombay Provincial Government. Dispensaries of local self-government bodies were started at convenient places in the state and they were called civil dispensaries, later all these dispensaries were classified under Zilla Parishad. In the 13th World Health Conference held in 1977, the health organization and its member countries set the health of society as the main goal of communication at a certain level, which became known as “Health for All” in 2000. It was recognized that primary health care is the key to achieving this goal.

    Various services provided by the health institution

    Maternal and Child Health:-

    1. Antenatal Care :-
      • Registration of all pregnant women (within 12 weeks)
      • At least five examinations during pregnancy :- First examination as soon as pregnancy is suspected, second examination (at 12 weeks), third examination at 4 to 6 months (26 weeks), fourth examination at 8th month (32 weeks) and fifth examination at 9th month (36 weeks
      • Comprehensive essential services like general check-up, weight, blood pressure, check for anemia, abdominal check, height, breast check, intake of folic acid in first trimester, intake of iron after 12 weeks, intake of folic acid tablets, dose of anti tetanus vaccine, treatment of anemia etc. (As per Guidelines for Health Nurses, Health Assistants)
      • Laboratory tests such as hemoglobin, urine protein and sugar tests.
      • Diagnosis and referral of at-risk pregnant mothers.
    2. Services during delivery :-
      • Emphasis on delivery in a health facility. (to motivate)
      • Childbirth by a trained person following the 5 rules of hygiene.
      • Providing prompt and appropriate referral services.
    3. Postnatal Care :-
      • Home visit at least 2 times after delivery. First home visit within 48 hours post delivery and second home visit between 7 to 10 days. If the child is underweight, five such visits should be made within 48 hours and at 7, 14, 21 and 28 days.
      • Initiate breastfeeding within half an hour after delivery.
      • Advice and counselling:- on diet and rest, hygiene, contraception, newborn care, infant and child feeding as well as venereal disease, HIV/AIDS etc.
    4. Postnatal Care :-
      • At least 2 postnatal home visits by sub center staff :- 1st within 48 hours post delivery and 2nd within 7 days post delivery.
      • Initiate breastfeeding within half an hour after delivery.
      • Initiate breastfeeding within half an hour after delivery.
    5. Health of child :-
      • Newborn care
      • Exclusive breastfeeding up to 6 months.
      • Immunization of all infants and children against vaccine preventable diseases.
      • Give 9 doses of A for life at 6-monthly intervals up to 5 years.
      • Prevention and treatment of malnutrition and diseases among children.

    Child Care :-

    1. Convenience and expert services for newborns
    2. Management of hypothermia and jaundice in the newborn.
      • Care of New Born Infant :-

    3. Acute care of sick children with Integrated Management of Illnesses in Newborns and Children (IMNCI).
    4. Care of common diseases in children.
    5. Promoting exclusive breastfeeding for the first six months after birth.
    6. Comprehensive immunization of infants and children against vaccine preventable diseases as per guidelines.
    7. Administer preventive doses of vitamin A.
    8. Prevention and control of diseases in children like malnutrition, germs etc.

    Family Planning and Contraception:-

    Providing health education, motivation and counseling to use appropriate family welfare methods.

    • Availability of family planning tools – contraception, copper, oral contraceptive pills, immediate contraception etc.
    • Follow-up services for eligible couples pursuing permanent family welfare approaches.
    • Counseling and appropriate referral services for safe abortion as needed.
    • Adolescent health care
    • Health education, counseling and referral services.
    • Support for school health care.
    • Comprehensive immunization of infants and children against vaccine preventable diseases as per guidelines.

    Therapeutic Services :-

    Treatment of minor ailments eg fever, diarrhoea, respiratory diseases, worm disorders, first aid in accidents and emergency situations.

    • Prompt and appropriate referral service.
    • Conducting at least one Health Day every month in Anganwadi with the help of Primary Health Center Medical Officer Asha Anganwadi Sevaka, Panchayat Raj Institutions, Self Help Groups.

    Record of Life Events :-

    Recording and reporting vital statistics (events) like births, maternal deaths, infant deaths (within 21 days)
    Primary Health Centre

    • a) Medical Services :-

      Out Patient Service :- 4 hours in the morning and 2 hours in the evening.

      24 Hours Emergency Care :- Proper management and first aid of injuries and accidents, bringing the patient out of danger of life before referral, giving proper care to dog bite, scorpion bite, snake bite and other emergency patients.

    • Referral Services:- Appropriate and prompt referral services for patients who require specialist services:-
    • Stabilization of patients
    • Providing appropriate ancillary services to patients during the referral journey.
    • Providing referral services from the vehicle of Prof. A. Center or from the available grant with Medical Officer, hired vehicle.
    • Care of common diseases in children.
    • Inpatient Services (6 Beds):-
    • Family Welfare Services

      • Educating, educating and counseling for adoption of appropriate family planning methods.
      • Providing contraceptives. eg contraception, oral contraceptive pill, emergency contraceptive pill, copper etc.
      • Permanent procedures like vasectomy, vasectomy/needleless vasectomy
      • Follow-up services for eligible couples using permanent methods of family planning such as surgery, where trained personnel and equipment are available, medical abortion services with the help of data technology and appropriate training.
      • Primary Health Centers in addition to the above services will also provide Janani Suraksha Yojana facilities.

        Management of Reproductive System Diseases / Venereal Diseases

      • Health education for prevention of reproductive system diseases/venereal diseases
      • Treatment of reproductive system diseases/sexual diseases.

        Dietary Services (in coordination with Integrated Child Development Services Scheme)

      • School Health :- Regular check-ups, appropriate treatment, referral services and follow-up.

      Adolescent Health Care

      • Life skills training, counselling, appropriate treatment.
      • To promote safe practices and sanitation.
      • Permanent diseases in that area. E.g. prevention and control of cold, black disease, Japanese, encephalitis etc.

        Epidemiological surveillance and control of epidemic diseases

      • Awareness and appropriate measures for abnormal health events
      • Disinfection of water bodies
      • Water quality should be tested using H2S field test developed by National Institute of Disease Control.
      • Encouraging sanitation including use of septic tanks, proper waste disposal.
      • Collection and reporting of vital statistics.
      • Health/Education Behavioral Change Messaging.
      • Implementing other national health programs including the National AIDS Control Programme.
      • Routine and immediate treatment facility services
      • These services should be available to patients who come to primary health centers and to patients who come from sub-centres or referrals from other places.

        This service includes the following.

      • Referral services to patients who cannot afford treatment or treatment at a primary health centre.
      • Providing inpatient treatment to those requiring hospitalization.

      At present, the rates of various viewers in the district are as follows.

      Antenatal services available to expectant mothers :- 98 percent

      Deliveries performed by a trained person :- 99.00 percent

      Male-Female Ratio (0 to 6 years):- 951 (Census 2011)

      Proportion of Protected Couples :- 74 percent

      National Family Welfare Programme

      1. RCH program
      2. National Insect Disease Control Programme
      3. Revised National Tuberculosis Control Programme
      4. National Leprosy Eradication Programme

      It is expected from Prof. A. Kendga that the P.A. Centers should do health promotion work and health services should be extended to rural areas. Health education is an essential (core) part of this programme. While doing health promotion work, it is expected to create health awareness among the people, and get their full cooperation and participation. Pvt. The basic services expected to be delivered by the center are as follows

      1. First Aid, Preventive Services
      2. Therapeutic care and referral services
      3. Reproductive and Child Health Services
      4. Health and nutrition education
      5. Guidelines for water quality monitoring and basic sanitation
      6. Prevention and control of locally existing diseases
      7. Collection of vital statistics and presentation of reports
      8. Implementation of various National Health Programmes.
      9. School Health Inspection/Anganwadi Inspection.

      11. Preliminary (basic) laboratory tests.
